TMS Smoke & Carbon Declaration

Hello TMS Residents,

As we are nearing the completion of the TMS Remodel, the attached Smoke and Carbon Monoxide Alarms Affidavit must be completed by each townhome owner to close out the permit.

 Please fill out the form apart from the Permit Number and Contractor Name and Signature, they will fill that part in before submitting. 

This form was also mailed to residents billing address.

To return the form you may,

1. Scan/Email to Maddie.Nicholas@fsresidential.com 
2. Mail form to Tahoe Keys POA, 356 Ala Wai Blvd, SLT, CA 96150 ATTN: Maddie Nicholas
